for the menus, you can put music files in that folder : C:\Program Files (x86)\Tennis Elbow 2013\Music\Menu .
The game will be able to play only music whose DirectShow Filter is installed on your Windows, so I guess you don't have a filter installed for M4A.

you don't need the v1.0g, you can play the Mod with the v1.0j. Don't install the music nor the sounds, it might help to make it work.

Ok, I found the source of the crash, it's because your Grunt #66 is actually this entry : Sound123 = Grunt-Female66a.wav, Grunt-Female66b.wav, Grunt-Female66c.wav And thus no sound is loaded for the Grunt #66 and it crashes when the game tries to play it. I'm not sure how you got the extra line feed...
