I just got a new computer and it has windows 11. When I try to open the mods.ini it opens it as a notebook and when I type in the mod I want to load it makes me save it as a notebook, but won't let me overwrite the mods.ini. Anyone know what to do?

You musn't write on mods.ini, let it as it.
If you want to add mod, just install it (or copy/paste) in the mods folder.

The instructions on the megapatchs is the write in the name of the mod patch in the mods.ini. I did it this way before on my previous computer with no issues. I believe the issue i'm having is that it's opening it as a notepad.

I don't have W11 so no idea what's going on.
Maybe try to install the game & the Mod in "C:\Tennis Elbow 2013\" instead of "C:\Program Files (x86)\Tennis Elbow 2013\".

I didn't do that and it works fine for me.
Add the mods files first in the main "page", second in the mod folder.
That is what i am doing.

I had to do "run as administrator" for it to work for some reason.
